Submitting a Cashless Claim
( For Local and Overseas Hospitals )

To receive cashless services, you can submit a fully completed Member Declaration Form and Pre-admission Form by attaching them and sending to healthclaim@daiichilife.com.mm.

You can also submit all required information online through Dai-ichi LOG Request Form

For local hospitals, the estimated processing time to obtain a LOG for cashless services is within 3 hours for emergency cases and within 6 hours for non-emergency cases. For overseas hospitals, the estimated processing time is within 7 working days.

Cashless Claims
Submitting a Cashless Claim for Loal and Overseas Hospitals

To receive cashless services, you can submit a fully completed Member Declaration Form and Pre-admission Form by attaching them and sending to healthclaim@daiichilife.com.mm.

For local hospitals, the estimated processing time to obtain a LOG for cashless services is within 3 hours for emergency cases and within 6 hours for non-emergency cases. For overseas hospitals, the estimated processing time is within 7 working days. Whenever a claimant requests overseas cashless service, the case fee amount will be deducted from his/her policy limit based on actual eligible hospitalization expenses.
