Health Claims

Claimants can submit health claim to Daiichi Life Myanmar by following channels.

    You can submit your claim by sending a fully completed health claim form, along with the relevant medical records and receipts, to healthclaim@daiichilife.com.mm .

Common Exclusions

Common Exclusions Not Covered Under Insurance Benefits ​

  • Pre-existing diseases
  • Waiting Period (30-day and 90-day)
  • Pregnancy-related conditions, including miscarriage
  • Mental and psychiatric illnesses
  • Alcohol and Drug Abuse related conditions
  • Routine medical check-ups and vaccinations
  • No dental and eye benefits unless related to accident
  • Not medically necessary treatment (e.g., cosmetics, weight loss)
  • All other cases that are excluded from insurance coverage can be viewed through this link (link).

Life Insurance Claims

Claimants can submit all life insurance claim to Daiichi Life Myanmar by following channels.

Submit your claim online

You can easily submit claims for death and total permanent disability benefits through the Dai-ichi Portal.

Cashless Claims

Submitting a Cashless Claim for Loal and Overseas Hospitals 

  • To receive cashless services, you can submit a fully completed Member Declaration Form and Pre-admission Form by attaching them and sending to healthclaim@daiichilife.com.mm.
  • You can also submit all required information online through Dai-ichi LOG Request Form
  • For local hospitals, the estimated processing time to obtain a LOG for cashless services is within 3 hours for emergency cases and within 6 hours for non-emergency cases. For overseas hospitals, the estimated processing time is within 7 working days. Whenever a claimant requests overseas cashless service, the case fee amount will be deducted from his/her policy limit based on actual eligible hospitalization expenses.

What are the required documents?

  • Duly completed “Claim Request Form”
  • Original Life Insurance Certificate (and Rider endorsement letter for rider benefits)
  • Other supporting documents (if required)
  • Confirmation Letter
  • Medical Reports
  • Treatment Records
  • Copy of Death Certificate (OR) Original Autopsy Report
  • Statement of attending doctor (physician/surgeon or relevant specialist) 
  • Recommendation letter of specialist doctor(s) who treated (or) Ward-in-charge professor from which Life Insured was admitted. 
  • Confirmation Letter/medical documents by hospital, clinic, laboratory, or a competent authority, as the case may be (if relevant) 
  • Original medical bills (for reimbursement benefit)
